Home Care Package, Levels 3 and 4

Funded by the Department of Social Services under the Commonwealth Home and Community Care Program, Homecare Package, Level 4 delivers high level packaged support to people who are frail aged (65 years or over), or if Aboriginal or Torres Strait Islander (45 years or over) or are a younger person with age related illness.

Eligibility is dependent on assessment by the Aged Care Assessment Team (ACAT). Packaged services can include (but are not limited to) case management, nursing care, personal care, domestic assistance, respite, transport, meal preparation, home modifications and maintenance and social support. The Level 4 package can also provide for the supply of goods and equipment such as continence aids.

Service is determined individually.
