Funded by the NSW Department of Health, this program provides short term case management (8 -12 weeks) and direct service support to enable people to return home safely after discharge from acute hospital care. The North Coast Area Health Service is responsible for all Transitional Aged Care packages and provide all case management for Transitional Aged Care recipients. The North Coast Area Health Service broker Community Care Options staff to provide community based supports.
Direct services include (but are not limited to) domestic assistance, personal care, transport, meals, respite and social support. The North Coast Area Health Service are funded to provide support with Physiotherapy, Occupational Therapy, Nursing support, Rehabilitation and other identified medical needs. Case Management addresses long term support options and aims to reduce the risk of possible readmission.
Eligibility to this service is determined by the Aged Care Assessment Team.
