The Out of Hospital Care (OHC) Program is funded by the NSW Ministry of Health and is comprised of three separate programs – Compacks, Safe and Supported at Home (SASH) and End of Life (EOL). The program aims to support you to discharge safely from NSW Health public hospitals and provide you with services you need to improve your recovery and reduce the risk of readmission to hospital.
Provides you with up to six weeks of support following discharge from hospital. Anyone requiring short-term, coordinated home care services is eligible.
Can support you if you have a progressive, life limiting illness or condition. EOL is designed to support you if you are in the palliative care phase of your illness or condition and require assistance with daily living tasks in order to die at home or remain at home for as long as possible. The package can also support family members or carers as they support you.
All OHC packages offer low to medium levels of care which can provide you with non-clinical case management and home care services. The services we provide to you are delivered on behalf of CareConnect and they remain responsible for the oversight of your package.
Personal Care. Supporting you to complete self-care tasks related to your hygiene and grooming
Domestic Assistance. Support with household tasks such as cleaning, laundry and linen support and unaccompanied shopping (where we go and purchase your groceries for you)
Social Support. One on one support to enable you to access the community or spend time with a Support Worker in your own home.
Respite. Support designed to enable you to take time for yourself and for the person you care for to engage in meaningful interactions with a Support Worker
Meal Preparation. Support to prepare meals in your own home according to your own unique preferences, dietary requirements and recipes
Transport. One on one support to enable you to access the community or attend appointments
